Club Application Form

A person interested in club membership fills out an online club application. Attach a scanned copy of their dog’s pedigree or a kennel registration certificate, if you own one. The club committee reviews membership applications individually.

After the application is approved, the KCHMPPP will invite the interested party to pay the registration and membership fee to the club’s account IBAN: SK57 0200 0000 0050 7991 0455.
After the specified fees have been credited, the new member will be included in the club’s list of members, of which the club’s membership base will be informed.

Each member is obliged to notify the club of any change in their surname, address, contact telephone number and e-mail address.

If a club member needs to change or add a different breed to the list of members than they stated in the application, they will send the club’s e-mail address a scanned copy of their new dog’s birth certificate.

By submitting the application, I accept the Breeding Regulations, as well as the Statutes of the KCHMPPP.
