Application form for breeding test

Instructions and procedure for completing the breeding test application form

Fill out the application no later than 7 days before the breeding test date. Applications received after this date will not be accepted and will be included in the next breeding test.

The fee for a KCHMPPP member is €20.
Fees for NON-KCHMPPP members are increased by 100%.
The fee must be paid to the club account IBAN: SK57 0200 0000 0050 7991 0455.

Add all attachments that you marked in the application. An application without complete attachments will not be accepted. We will inform you about the time schedule by e-mail no later than 3 days before the breeding test. Failure to participate in the breeding test for any reason is not a reason for a refund of the fee.

Payment confirmation /mandatory/
Pedigree scan (for imported dogs with SPKP number) /mandatory for all breeds/ Scan of the judgind evaluation from the club or specialty KCHMPPP show /mandatory for all breeds/
PRA scan of the clinical ophthalmological examination result. For the Puli breed, genetic testing is possible /mandatory for all breeds/
Scan of the patella luxation examination result /mandatory for Puli, Pumi, Mudi/
Scan of the DBK and DLK examination result /mandatory for all breeds/
Scan(s) of other examination results or color locus tests /optional/
