Breeding permission request form

The owner of a breeding bitch – i.e. a bitch that has passed and met the breeding conditions – sends their application online. They attach the names of three stud dogs, one of whom they would like to breed their bitch to.
The application is accompanied by double-sided photocopies of documents – the pedigree of the chosen breeding dog, proof of passing the breeding test or inclusion in the list of breeding dogs (unless this is indicated directly on the pedigree) and a confirmation of completed health examinations.
The electronic application for breeding is sent to the club at least 3 weeks before the planned breeding. Based on this application, the breeding advisor will issue a breeging permision letter.
